INFORMATION ABOUT THE VOLUNTARY INSURANCE

 

If you are interested in the voluntary insurance, please take the following steps:

 

Step 1.

Bring your passport, student ID card and the application to the regional branch of the National Health Fund (NFZ), located at 3 Palacowa Street, 15-042 Bialystok. You can ask your Mentor to assist you with that.

 

Step 2.

After submitting the application and documents, you will receive a contract for signature.

 

Step 3.

After signing the contract for voluntary insurance with the NFZ, you should submit the contract to the Polish Social Insurance Institution (ZUS), located at 29 Mlynowa Street, 15-404 Bialystok, within 7 days from the date when the contract was signed. You will receive the bank account number, on which you will have to make the monthly payments for the insurance.

For each month of being covered by the health insurance, you are obliged to submit the ZUS DRA settlement statement and pay contributions to the ZUS account in the amount of 55,80 PLN.

 

Important information

 

In case of emergency, medical services are given immediately and without any referral. In case of direct danger of life, you should call an ambulance – 999 or 112.

 

In case of subjective feeling of danger or in case of emergency, you can benefit from emergency help in the Emergency Department (Szpitalny Oddział Ratunkowy – SOR) without any referral.

 

The Emergency Room (Izba Przyjęć) takes care of those who are in a life-threatening condition, for example caused by an accident, injury or poisoning.

Remember that the Emergency Department is the place where medical advice is given. During the day, medical services are given by a family doctor or a medical specialist in an ambulatory mode.

Each provider within the confines of the contract agreement with the NFZ is obliged to mark the NFZ logo in front of the building. This sign means that if you are insured, you can benefit from medical services there. That is why such indicator should be placed in a visible and available place.

 

Night and festive medical care included in the found

 

Night and festive medical care provides you with basic medical services Monday to Friday from 6 p.m. to 8 a.m. and also on Saturdays, Sundays and bank holidays from 8 a.m. to 8 a.m. the next day. These services are free and you do not need any referral.
